• For the first time client, please print and fill out this form and bring to your first session with me! (If you don’t have a printer, don’t worry. You can fill out the form at my studio, too. Please come at lease 5 ~10minutes earlier than your appointment time so that I can assess your condition and customize a massage for you. Also I could use all of your appointment time for a massage!)
